After a project, a team often remembers only the final conclusions: what worked and what did not. By the next decision, the circumstances may have changed, so the previous answer may no longer apply. Saving only a conclusion loses the conditions that made it valid.¹ A useful review asks what evidence was available, which explanations were considered, and why other options were ruled out. Link judgments to the original evidence, reasoning, and limits so new feedback can help revise them.² Notes should help us test our judgments again, rather than merely remember an answer.
